Promotional opportunity available in our Community Residential Program. The Community Program Coordinator (CPC) provides overall direction and coordination for the Person-Centered Plan, including goals and objectives, advocating for the rights of individuals, and serving as the leader of the New Hope team process. This position prepares reports for all ID team meetings as well as other documents as required. This person will work closely with Community-Based Case Managers.
Must become proficient with Set Works. Will monitor staff delivery of services and the progress of individuals served. Good communication, writing and reading skills are necessary. Must be professional, dependable, flexible and able to work independently.
Applicants should have an Associate Degree in a related field such as Human Services, Developmental Disabilities or Intellectual Disabilities or equivalent in years of experience in related field.
Required Experience: One year of previous experience in ID/DD or health care setting. Preferably, applicants would have 3 to 5 years of previous experience in a similar agency. Applicants will need to complete the Medication Manager course, must have a valid driver's license and be insurable to drive with New Hope.
